How do I know if I can get a card?

Do You Qualify?

New Hampshire has a growing list of qualifying conditions for therapeutic cannabis.

Conditions I commonly certify include:

  • Generalized Anxiety Disorder
  • Post-Traumatic Stress Disorder (PTSD)
  • Chronic pain
  • Insomnia
  • Opioid Use Disorder
  • Cancer, HIV/AIDS, glaucoma, and other serious diagnoses
  • Autism Spectrum Disorder (under specific guidelines)


Patients 21 and older may also qualify under any debilitating or terminal condition where, in the certifying provider’s clinical opinion, the potential benefits of therapeutic cannabis would likely outweigh the potential health risks. Find out how this works

How the Process Works

1. Book Your Appointment

Schedule your certification visit online. Available via telehealth statewide or in person at our Manchester, NH office.

2. Meet With Me

We’ll review your health history, confirm your qualifying condition, and I’ll complete the required written certification for your application.

3. Submit to the State

You’ll submit your completed application, written certification, proof of NH residency, and the $50 state application fee to the NH Department of Health and Human Services. Applications go by mail or in person.

4. Get Your Card

Once approved, your NH cannabis registry ID card arrives by mail within approximately 3 weeks. Then you’re ready to visit any licensed Alternative Treatment Center in the state.
